Terminal illness benefit claim
The insured life must be suffering from a disease which in the opinion of an appropriate consultant, and supported by Momentum's Medical Officer, is likely to lead to death within 12 months from the date of notification.
This special feature is available free of charge and gives policyholders the right to claim the full benefit amount of the death benefit and/or critical illness benefit if the insured life becomes terminally ill.
If a valid claim is admitted, the claim amount will be paid to the following people in the following order:
- The cessionary (if any).
- The policyholder.
Terminal illness benefit claim requirements
- The terminal illness claim form (CLAIM027) completed by the treating specialist.
- The POPIA consent form (CLAIM026) completed and signed by the claimant or authorised representative of the claimant.
- A copy of the insured life’s identity document.
- An up-to-date confidential medical report from the treating specialist, which must include the full diagnosis and prognosis.
- Copies of all the relevant diagnostic tests/reports.
- Proof of banking details, in the name of the claimant (natural person, minor, deceased estate, legal entity), not older than 3 months on a bank letterhead or that has a bank stamp on it.
